OMB Approves Revised Forms 486, 500, 479 |

On August 10th the Office of Management and Budget (OMB) approved revisions to Forms 486, 500, and 470 which update the Privacy Act statement and Paperwork Reduction Act burden statement contained on each form.
The new Form 486 will also include a new certification that "certain steps have been taken prior to the commencement of service." Specifically that, "Libraries receiving Internet access and internal connection services…are enforcing a policy of Internet safety and enforcing the operation of a technology prevention measure."
The SLD has not indicated when the new forms will be made available, or when the old versions of the forms will be phased out of use. |
