USAC has extended the deadline for E-rate applicants who have yet to certify their Form 471 or submit their Item 21 Attachments. While applicants are technically required to have this information submitted by the time the Form 471 window closes, the SLD has provided applicants until Wednesday, May 9, 2012 to complete the FY 2012 Form 471 submission process. Uncertified applications or those missing Item 21 Attachments after May 9 will be considered “out-of-window” and will likely not receive funding for FY 2012.
