On November 23, 2013, the Federal Communications Commission issued a Public Notice seeking comment on proposed revisions to the FCC Form 499-A, Form 499-Q, and associated instructions. The FCC Forms 499-A and 499-Q are filed by providers of telecommunications and Interconnected VoIP services. Proposed versions of the Forms and instructions may be found at the following links:
