Update Required to E-rate Payment Form

On Monday, August 12, USAC began notifying program participants that an update will be required to their FCC Forms 498.  The update needed for compliance with the Digital Accountability and Transparency Act of 2014 (DATA Act), and applicants and vendors must now supply a “business type” on the Form. A list of options for “business type” may be found on USAC’s website.
Applicants update their FCC Form 498 via the E-rate Productivity Center (EPC.) While the General Financial Contact may queue updates to the 498, it must be certified by a School or Library Officer in order for the changes to take effect. In addition, if the existing Form 498 does not include a Dun & Bradstreet number (DUNS), it will be required during the update (DUNS numbers may be requested at https://www.dnb.com)  Applicants who use the SPI method of invoicing exclusively are not required to have a FCC Form 498 on file (or make this update.)
Service providers can update their Forms 498 via USAC’s E-File system. Because the update requires the Form 498 to be recertified, a Company Officer must complete the process.
