On August 19, 2013 the FCC released a Public Notice alerting service providers of the revised form. While the changes to the form only affect those service providers who participate in the Rural Health Care Program, any new service provider that wishes to participate in the E-rate program would also need to use the August 2013 Form 498. Any E-rate service provider that wishes to make any changes or edits to their information must also use the new form as well.
