SLD Implements Online Filing for Form 486

The Schools and Libraries Division has implemented a Form 486 application that can be completed online.

The Form 486 is submitted by an applicant to notify the SLD that it has begun using E-rate-supported services and its vendor can be paid. The Form 486 must now be submitted within 120 days of the date of the funding commitment letter or the service start date, whichever is later. The online form can only be used for funding requests for the 2001 and 2002 funding years.

The new online form is in scannable PDF format, and requires the use of Adobe Acrobat 5.0 or higher. Currently, the form must be completed in one session; it cannot be saved and completed at a later date. The application can now be certified either on paper or through the SLD’s electronic certification procedure. However, if an applicant wants to retain a copy of the form, it must print the copy before it chooses to exit or use electronic certification.
