Draft 2014 FCC Forms 470 and 471 Submitted for Approval

The FCC has submitted draft versions of the E-rate Form 470 and Form 471 to the Office of Management and Budget (OMB) for emergency approval. The OMB must approve the FCC forms before they can be officially used for the E-rate application process. Under the emergency approval process, the forms will be open for comment to the OMB for 30 days instead of the standard 60 days.

Upon approval, these new forms will be utilized by E-rate applicants for the Funding Year 2015 filing window, and reflect E-rate modernization changes from the recent 7th Report and Order.

The FCC issued notice in the Federal Register on September 22nd, 2014, with instructions on how to comment on the forms. Comments are due by October 22nd.
