FY2013 Form 486 Deadline Approaching

Applicants that received a Funding Commitment Decision before July 1, 2013, need to file an FY2013 Form 486 by October 29, 2013. Missing this deadline may lead to a loss or reduction in funding.

A Form 486 must be postmarked or received no later than 120 days after the service start date shown on the Form 486 or 120 days after the date of the FCDL, whichever is later. If you received your FY2013 FCDL before July 1st and you are currently receiving services, the deadline to file the Form 486 is October 29, 2013.

The Form 486 informs USAC that:

  • Services listed on the FCDL have started and invoicing can begin,
  • Your technology plan has been approved by a certified Technology Plan Approver (required for Priority Two applications only), and
  • You are in compliance with the Children’s Internet Protection Act (Not applicable for application that are solely telecommunications, including VoIP)

Applicants must remember to submit your Form 486 by the deadline, as your funding may be affected if filed late. Forms 486 filed after the deadline will be subject to a reduction in funding.  A look at the funding data available shows there are approximately 4,852 individual funding requests that were funded before July 1, 2013, that currently do not have an approved Form 486 service start date.  These FRNs represent $27,219,117.91 in approved funding.
